1915(i) Housing Support Services (Medicaid)

FOCUS is an approved provider of Wisconsin’s 1915(i) Medicaid benefit, offering housing-related services to individuals who are experiencing or at risk of homelessness. These services include:

​

• Housing Consultation: We work with clients to develop a personalized housing plan based on their needs, preferences, and barriers to finding and sustaining housing.

• Finding Housing: Our staff helps clients search for housing, complete applications, and navigate the move-in process.

• Sustaining Housing: Once housed, we provide coaching and coordination to help clients maintain stability, resolve conflicts, and build independent living skills.

• Housing Assistance: For clients leaving temporary shelters or healthcare facilities, we assist with essential housing expenses like security deposits, utilities, and basic furnishings.

Supportive Housing & Eviction Prevention

FOCUS partners with local Continuums of Care and the City of Madison to deliver:

​

• Case management for permanent supportive housing

• Rapid rehousing supports

• Housing search and landlord mediation

• Legal resource navigation and eviction diversion programs​​​​
